identified. It will provide you with what you need most — high level physician support, navigation, liaison

and medical resources. You can expect:

Rapid response, with personalized, professional care management before, during, after diagnosis,

and throughout treatment and recovery. One call does it all and you will have a single point of

contact going forward.

The opinion of a world-class, independent expert from a U.S. Centre of Excellence to confirm the

diagnosis and counsel for the best plan of action prior to beginning treatment.

Care management oversight throughout the treatment and follow-up including during hospital stays.

Services are provided by MedExtra, a Canadian company that helps people resolve their health problems with

nurses, physicians and a network of affiliated physicians and clinics. They recognize that many people desire

more time, information, and medical resources than the healthcare system and their physicians are equipped

to provide.

Your services include:

Remote Second Opinion (RSO).

RSO is a tailored service for individuals when the need for a second opinion

is identified. You are provided with what you need most – high level physician support, navigation, liaison and

medical resources with thorough assistance before diagnosis, during the RSO process, and before and after

treatment.

How it works:

You can contact MedExtra directly at 1.866.514.1906. You will need to provide:

• Employer name

• Group ID number

When you call, you will speak with a Client Care Advocate, and a Nurse Care Manager who will, through

discussion, gain an understanding of the case so an action plan can be created to diagnose the problem, or to

find treatment. Working closely with one of the Physician Team Leaders, the Care Management team will

prepare and present the plan to you within 72 hours.

Note: You and your eligible dependents are entitled to one Remote Second Opinion per calendar year per

covered condition.
